Referral Process
We aim to make referrals simple and efficient:
1.Initial Referral
Please use our Referral Form to provide patient details and clinical background.
2. Consultation & Assessment
We contact the client directly for an assessment. Patients attend a one-on-one assessment (virtual or in-person) using standard tools such as the Oswestry Disability Index and pain scales.
3. Personalised Plan
Based on the assessment, we recommend a tailored package (e.g., 6-week Intro, 12-week programme, or ongoing membership).
4. Ongoing Monitoring
While clients attend live classes multiple times a week as part of their programme, 1:1 check-ins are scheduled at key points — typically at the start, mid-point, and end of the referral period (e.g. Week 1, Week 3, and Week 6 or 12) — to review progress, adjust their plan, and provide personalised support.
5. Professional Updates
With the client’s consent, we provide optional progress reports and feedback to referring professionals. This ensures coordinated care and keeps you informed of their engagement, improvements, and any further support they may need.
